CHINO AREA PARTNERSHIP

Our Mission for the Chino Area Partnership Tri-City Medical Home Project is to improve the quality of pediatric health care and related services for children with special health care needs.  Our approach is to build upon the existing infrastructure in our community to develop a network that includes parents, primary care physicians, specialty care providers, care coordinators and community members. 

We have developed the "Welcome To Holland" Bags based on a poem written by Emily Perl Kingsley. In this poem she describes what it's liketo parent a child with special needs and likens it to taking a vacation in an unexpected destination. 

Our "Welcome to Holland" Bags carry on the travel theme and are filled with useful resources families with children newly diagnosed or new to the Tri-City area. The most important resource you will find is the list of "Tour Guides."  These are the parents of children who have traveled this road before and have volunteered to act as a personal guide to you and your family.
